By booking:  I am 18 years of age or older. I understand that energy work is a gentle, sometimes hands on, techniques used for stress reduction and relaxation.  I understand that energy work practitioners neither diagnose conditions, prescribe or perform medical treatment, prescribe substances, or interfere with the treatment plan of a licensed medical professional. 

I understand that energy work does not take the place of medical care.  It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have. 

I understand that energy work may complement, but should not be used as a substitute or replacement for any medical or psychological care I may be receiving.

I understand if I am under 18, I MUST be accompanied by an adult, and/or will sign a waiver acknowledging that the sessions are not intended to diagnose, prescribe, or treat any disease, conditions, or injury nor constitute or substitute for medical or psychological remedies.

I understand that individual results may vary by client; therefore their effectiveness depends on the client and no guarantee can be made regarding the results of its use. 

I understand and agree Reiki & Soundwaves, its owners, claims no responsibility whatsoever to any person or entity for any liability, loss, or damage caused, or alleged to be caused, both directly and indirectly, as a result of the session I received today.  I also agree to contact my energy work practitioner in the event of any issues that should arise from the session I received today.  I understand that all information shared with Reiki and Soundwaves will be held in the strictest of confidence. 

All sessions are fully non-refundable.
